Charm Patient Portal

At Rejuvenate Muncie, we use CHARM PHR as our Patient Portal and Electronic Medical Record system. Please use Charm to complete any of the following task, and or request:

  • Check upcoming appointments

  • Make a new appointment

  • Send a message to the provider

  • Review current medication’s

  • Review recent visit information

  • Complete questionnaires

FAQS

I am trying to access Charm for the first time via my phone, but every time I enter the security code and change screens it resets. What can I do?

If it is your first time accessing your Charm patient portal account, it is best to do this via a laptop or tablet device as the security code needed to activate will reset if you are on your cell phone and switching between tabs.

I am out of medication, and my next appointment is not for several weeks, how do I request a refill?

If your appointment is weeks out and you have not missed a follow-up appointment, you may go into Charm under the medication tab and select the medication. You will select refill request. Please allow 48 hours for a response on all refill requests. When we respond, we will let you know when the medication will be ready.

Do I have to pay a deposit every time?

Depending on the service, you do have to pay a deposit when scheduling the appointment. This is the same for a new and current patients. Please note, some appointments require a deposit, but all will require you to enter your card information to have it on file - this does not mean your car is being charged.
