
Direct Primary Care
At Rejuvenate Muncie, we want to address each patient as an individual and as a whole person - not just parts. Direct Primary Care (DPC) is a different approach to traditional health management. While traditional practices are often regulated by insurance companies and overburdened by trying to fit as many patients in as possible, we do not have to worry about those issues.
Reasons to choose Direct Primary Care (DPC):
Insurance - Whether you have insurance or not, you can use DPC - for those with insurance, many companies recognize these visits and offer a discount; for those without insurance, DPC offers an alternative to health insurance regardless of work status.
Price - With DPC, you will pay a flat fee-for-service, making it easy to budget your health. You may also choose to pay per visit, monthly or annually, choosing an option that works best for you.
Better Health - By removing the constrictions of traditional practices, we are able to offer extended appointment times, increased communication through text and patient portals, in-person and Telehealth options, and we focus on a holistic-style of health. We recognize the place and need for traditional medicine, but we also incorporate a functional approach and take an alternative approach to lab testing and interpretation to find the root cause and treat symptoms. Every patient is different, therefore every patient may not fit the same “box”.
Membership
Initial Consultation
60 minutes
$200 for new patients 18+ $100 for new patients 17 and under $100 for current Total Membership enrollees
During your initial consultation, the provider will review your medical history and conduct a health assessment screening. She will then discuss your concerns and wellness goals, review available treatment options, and discuss the potential treatment plan.
Your initial consultation can be completed via video conference or in-person.
Treatment options may include: supplements, medication, combination-therapy, and more.
You will be given an order for lab work during your appointment. All patients are required to have initial labs to initiate treatment and lab work may be repeated at various stages during the treatment plan. Labs are patient and treatment-specific and the cost of labs are not included in the consultation cost.
Additional testing may be needed and will be discussed on a patient-specific basis.
*You must have your labs and testing completed (and submitted to the office if done off-site) at least 48 hours prior to your next appointment or you will be rescheduled.

We will work together to create an individualized treatment plan. All treatment plans include:
Nutritional and fitness guidance
Lifestyle modification coaching
Goal setting
Initiation of treatments
This may include a daily medication, daily supplement, injections, patch, or more
This is individualized to each patient
The frequency of follow up appointments very on patient needs; some may need to be monthly, every 3 months, 6 months, or annually. This will be discussed after your initial consultation. Depending on the need, these appointments may be in-person or virtual.
There is no appointment limit with these plans.

*Acute care visits/lab visits are available to ANY patient on the Total Rejuvenate membership at the per visit price.

Appointments may be scheduled in advance or if there is an acute need, we may schedule a same day appointment if there is an appointment time available. Priority is given to those with annual or monthly subscriptions.

Labs are 50% off with monthly or annual memberships. You may also use your personal insurance or pay via the clinic for the below labs.

Miscellaneous
Claims/PAs/Forms
If you need a Prior Authorization or Paperwork completed by the office, this will be $100 per submission.
**We do not submit prior authorization for weight loss or hormone therapy.
Vaccines
We do not offer vaccines. Please visit the local health department.
Additional Tests/Imaging
When special testing is needed and you do not have a health insurance plan, we will refer you to the nearest low-cost imaging center. This may be Indianapolis. Keep in mind that some testing is required (such as mammograms) and we want to be able to ensure you are cared for AND reduce the cost as much as possible.
Prescriptions
We now have in-office prescriptions available that can be dispensed instead of sending to a local pharmacy. Most are 30 day supply or shorter. For longer prescriptions, we will also use a low-cost pharmacy for at home delivery. We will also continue to use our compounding pharmacies.
These costs are separate from the membership fee.
FREQUENTLY ASKED QUESTIONS
Do you accept insurance?
We do not accept insurance at Rejuvenate Muncie. You may use your HSA/FSA/HRA in your office to make payments. You may use your insurance for labs, testing, and prescriptions at commercial pharmacies however this is completed at your insurance companies’ discretion.
NOTE: Medicaid and Medicare patients absolutely may not submit for reimbursement. This will result in dismissal from the clinic.
Are medications covered in the price?
Medications are not covered in your annual, monthly, or per visit pricing however you have options:
We have many prescriptions available in the office
You can use your insurance at a commercial pharmacy
We can send prescriptions to lower-cost pharmacies including TopRx and Cost Plus
We use many compounding pharmacies with reduced pricing options and many of our alternative treatment options will come from these pharmacies
Our supplements come from Fullscript and will be offered at a discount price
I am on one of your other programs (weight loss, hormone) - does this cover that?
DPC is a separate fee from the hormone and weight loss programs. If you decide to participate in more than one program at Rejuvenate Muncie, you will be offered a discounted rate as long as you are participating in multiple programs.
